Those Communities of Faith seeking to re-licensing of a Sacraments Elder for the Pastoral Year, July 01, 2021 to June 30, 2022 should complete the appropriate form and forward it to the Pastoral Relations Minister as noted on the form prior to the deadline of June 18, 2021. Please note that at least one of the three required signatures should appear on the down loaded form and the other approvals can be given through a direct email to the Pastoral Relations Minister (jhare@united-church.ca). The motion by the governing body of the Community of Faith requesting the re-licensing must have occurred at some point in 2021. Please click here to access the form.
